The exact title.
The date you wish to use it plus an alternate date.
Source, price, and catalog number if you have not used the film previously.

The faculty service area in Room 332 Hartman Hall contains a collection of catalogs and informational material pertaining to 16 mm films, filmstrips, and audio/video tapes either through rental or free loan.
